Tutuculuk i\u00e7in uygun olan en basit kro\u015fe kullan\u0131lmal\u0131d\u0131r. Kro\u015fenin stabilizasyon \u00f6zelli\u011fi iyi olmal\u0131, fonksiyonel streslerle aktive edilene kadar pasif kalmal\u0131 ve destek di\u015fe tork iletmeden protez kaidesinin s\u0131n\u0131rl\u0131 miktarda hareketine izin verecek esnekli\u011fe sahip olmal\u0131d\u0131r.\r\n\r\n<strong>Direkt tutucunun yerle\u015fimi:<\/strong> Hareketli b\u00f6l\u00fcml\u00fc protezde kullan\u0131lacak direkt tutucu say\u0131s\u0131 ve yeri belirlenirken, tutuculuk sistemi ve fulkrum ekseni g\u00f6z \u00f6n\u00fcnde tutulur. T: Tutucu, \u0130T: \u0130ndirekt tutucu.\" width=\"340\" height=\"309\" \/><\/a> <strong>\u015eekil 8-9.<\/strong> Direkt tutucunun yeri belirlenirken, tutuculuk sistemi ve rotasyonun d\u00f6nme ekseni g\u00f6z \u00f6n\u00fcnde tutulur. \u0130ki direkt tutucunun kullan\u0131ld\u0131\u011f\u0131 vakalarda fulkrum ekseni olu\u015ftu\u011fundan, indirekt tutucu kullan\u0131larak, direkt tutucular\u0131n etkisi artt\u0131r\u0131l\u0131r. <strong>T:<\/strong> Tutucu, <strong>\u0130T:<\/strong> \u0130ndirekt tutucu.[\/caption]\r\n\r\nEn ideal se\u00e7im, \u00fc\u00e7 adet tutucunun \u00fc\u00e7gen \u015feklindeki yerle\u015fimle birbirinden m\u00fcmk\u00fcn oldu\u011fu kadar uzak tutulmas\u0131d\u0131r. \u00dc\u00e7\u00fcnc\u00fc tutucu, di\u011fer iki tutucu aras\u0131ndaki rotasyonu engeller ve tutuculuk sistemini daha stabil k\u0131lar. Destek di\u015fler \u00fczerindeki mevcut and\u0131rkatlar veya and\u0131rkat derinlikleri ortalaman\u0131n alt\u0131nda ise ve rehber d\u00fczlemler k\u0131sa ve say\u0131s\u0131 az ise daha fazla say\u0131da direkt tutucu kullan\u0131lmas\u0131 gerekir. Ancak ayn\u0131 d\u00fczlem \u00fczerinde ikiden fazla tutucu kullan\u0131lmas\u0131 mekanik a\u00e7\u0131dan bir avantaj sa\u011flamad\u0131\u011f\u0131 gibi, plak birikimi ve destek di\u015fte olu\u015fturaca\u011f\u0131 stresler a\u00e7\u0131s\u0131ndan sak\u0131ncal\u0131d\u0131r. Serbest sonlu hareketli b\u00f6l\u00fcml\u00fc protezlerde oldu\u011fu gibi, sadece iki destek di\u015fin mevcut oldu\u011fu vakalarda zorunlu olarak iki adet direkt tutucu kullan\u0131l\u0131r; anterior modifikasyon bo\u015fluklar\u0131n\u0131 \u00e7evreleyen sekonder destek di\u015flere, estetik sak\u0131nca nedeniyle kro\u015fe yerle\u015ftirilmez.\r\n\r\nKennedy S\u0131n\u0131f I vakas\u0131nda, anterior k\u0131sa modifikasyon bo\u015fluklar\u0131n\u0131n k\u00f6pr\u00fc restorasyonu ile kapat\u0131lmas\u0131 da bir planlama se\u00e7ene\u011fi olabilir. Modifikasyonu \u00e7evreleyen destek di\u015flerin ante kural\u0131na uygun destek \u00f6zelli\u011fi sergilemesi; desteklerin klinik kron boyutlar\u0131n\u0131n sabit restorasyona uygun olmas\u0131; destek di\u015flerin madde kayb\u0131, restorasyon veya malformasyon, renklenme, malpozisyon gibi estetik problemlere sahip olmas\u0131; desteklerin endodontik tedaviye sahip olup sabit restorasyonla g\u00fc\u00e7lendirme gereksinimi do\u011furmas\u0131 ve hastan\u0131n estetik beklentisinin y\u00fcksek olmas\u0131 gibi ko\u015fullar\u0131n herhangi birinin mevcut olmas\u0131, planlamada anterior k\u0131sa di\u015fsiz bo\u015fluklar\u0131n k\u00f6pr\u00fc restorasyonu ile kapat\u0131lmas\u0131 se\u00e7ene\u011finin de de\u011ferlendirilmesini gerektirir. Kennedy S\u0131n\u0131f II ve III vakalar\u0131 i\u00e7in de ayn\u0131 durum ge\u00e7erlidir.\r\n\r\nPlanlamada ilave destek ve tutuculuk i\u00e7in ihtiya\u00e7 duyulmayan, premolar b\u00f6lgedeki k\u0131sa di\u015fsiz bo\u015fluklar da yukar\u0131daki kriterler dahilinde k\u00f6pr\u00fc restorasyonu ile kapat\u0131labilir.\r\n\r\nDi\u015fsiz sahalar\u0131n k\u0131sa, and\u0131rkat ve rehber d\u00fczlemlerin uygun oldu\u011fu vakalarda ise diagonal olarak yerle\u015ftirilen iki adet direkt tutucu yeterli tutuculuk sa\u011flayabilir; yine bu durumda da kro\u015fe kullan\u0131lmayan di\u015flere yerle\u015ftirilen okluzal t\u0131rnaklar indirekt tutucu olarak g\u00f6rev yapacak ve protez stabilizasyonunu artt\u0131rarak, direkt tutuculu\u011fu destekleyecektir.\r\n\r\nDi\u015fsiz sahalar\u0131n \u00e7ok k\u0131sa ve rehber d\u00fczlemlerin \u00e7ok uzun oldu\u011fu vakalar d\u0131\u015f\u0131nda, tek bir direkt tutucunun kullan\u0131lmas\u0131 yeterli tutuculuk sa\u011flamaz. Bu durumda doku \u00f6rt\u00fcc\u00fcl\u00fc\u011f\u00fc maksimum kullan\u0131larak (tam palatal plak yard\u0131m\u0131yla) direkt tutuculu\u011fa yard\u0131mc\u0131 olunur.\r\n\r\nKro\u015felerin konumu belirlenirken stresi maksimum \u015fekilde kontrol etme \u00f6zelli\u011fi g\u00f6z \u00f6n\u00fcnde tutulmal\u0131d\u0131r.\r\n\r\n<strong>Direkt tutucunun tipi:<\/strong> Tutucular\u0131n yerle\u015fimine karar verildikten sonra, kullan\u0131lacak olan direkt tutucu tipi se\u00e7ilir.\r\nDirekt tutucu se\u00e7iminin ba\u011fl\u0131 oldu\u011fu kriterler:\r\n<ul>\r\n \t<li>Destek di\u015fin durumu,<\/li>\r\n \t<li>Esneklik miktar\u0131,<\/li>\r\n \t<li>Ekvator hatlar\u0131n\u0131n etkili kullan\u0131m\u0131,<\/li>\r\n \t<li>Tutucu alanlar\u0131n lokalizasyonu,<\/li>\r\n \t<li>And\u0131rkat derinli\u011fi ve miktar\u0131,<\/li>\r\n \t<li>Yumu\u015fak doku konturu,<\/li>\r\n \t<li>Estetik.<\/li>\r\n<\/ul>\r\n<strong>Destek di\u015fin durumu:<\/strong> Periodontal deste\u011fi kemik kayb\u0131ndan olumsuz y\u00f6nde etkilenen destek di\u015flerin varl\u0131\u011f\u0131nda, b\u00f6yle bir di\u015fe tutucu yerle\u015ftirmekten ka\u00e7\u0131n\u0131lmal\u0131 veya zorunlu hallerde, daha esnek bir tutucu (b\u00fckme kro\u015fe) se\u00e7imi ile y\u00fck\u00fc s\u0131n\u0131rland\u0131r\u0131lmal\u0131d\u0131r.\r\n\r\n<strong>Esneklik miktar\u0131:<\/strong> Belli bir esneklik gerekli olmas\u0131, tutucunun \u015feklini ve kullan\u0131lacak materyali belirler. Daha uzun veya ince tutucu kolu olan kro\u015felerle daha fazla esneklik elde edilir. D\u00f6k\u00fcm yerine b\u00fckme kro\u015feler kullan\u0131larak, daha esnek bir kro\u015fe plan\u0131 sa\u011flan\u0131r.\r\n\r\n<strong>Ekvator hatlar\u0131n\u0131n etkili kullan\u0131m\u0131:<\/strong> Ekvator hatt\u0131, protezin esnek olmayan, rijit k\u0131s\u0131mlar\u0131n\u0131n bu hatt\u0131n \u00fczerinde planlanmas\u0131 gere\u011fi a\u00e7\u0131s\u0131ndan \u00f6nemlidir. Ekvator hatt\u0131n\u0131n alt\u0131ndaki and\u0131rkat alan\u0131 i\u00e7inde, protezin sadece esnek k\u0131s\u0131mlar\u0131 planlan\u0131r. Hareketli b\u00f6l\u00fcml\u00fc protezin tek esnek par\u00e7as\u0131, kro\u015fenin tutucu olan u\u00e7 k\u0131sm\u0131d\u0131r. Gerekti\u011finde protezin giri\u015f yolu de\u011fi\u015ftirilerek, and\u0131rkatlar\u0131n boyutu ve yeri de\u011fi\u015ftirilebilir (\u015eekil 8-10); veya yeterli and\u0131rkat elde etmek amac\u0131yla destek di\u015fin kronla restorasyonu d\u00fc\u015f\u00fcn\u00fclebilir. Destek di\u015fin minesi \u00fczerinde kro\u015fe i\u00e7in and\u0131rkat olu\u015fturulmas\u0131 da bir y\u00f6ntem olabilir (<a href=\"https:\/\/ekitap.ankara.edu.tr\/hareketli-bolumlu-protezler\/part\/10-bolumagiz-hazirligi\/\">Bkz. Direkt tutucunun tipi daha \u00f6nceden model \u00fczerinde tutucu alanlar\u0131n belirlenmesiyle ve hareketli b\u00f6l\u00fcml\u00fc protezi okluzal y\u00f6nde yerinden \u00e7\u0131kar\u0131c\u0131 olan de\u011fi\u015fik b\u00fcy\u00fckl\u00fckte ve y\u00f6ndeki kuvvetlerin de\u011ferlendirilmesiyle se\u00e7ilir.\r\n\r\n<strong>Ekvator hatt\u0131 s\u0131n\u0131f\u0131na g\u00f6re and\u0131rkat konumlar\u0131 ve kro\u015fe tipleri:<\/strong> Model analizi esnas\u0131nda, destek di\u015fin kontur y\u00fcksekli\u011fi \u00e7izici u\u00e7 yard\u0131m\u0131yla belirlendi\u011finde, ekvator hatt\u0131 ortaya \u00e7\u0131kar. Ekvator hatt\u0131, di\u015fi \u201cand\u0131rkat alan\u0131\u201d (\u00e7izginin alt\u0131nda kalan b\u00f6l\u00fcm) ve \u201cand\u0131rkats\u0131z alan\u201d (\u00e7izginin \u00fczerinde kalan b\u00f6l\u00fcm) olmak \u00fczere ikiye ay\u0131r\u0131r.
